What does this service provide?
- Caring for people at the end of their lives ensuring they can remain in their own home if this is their choice. This might involve ensuring the patient has the right drugs so they are free of pain, that their last months, weeks or days are as comfortable as possible and that the family are supported before, during and after a loved one’s death

- Health promotion and education to encourage patients to be self-caring.
A team of qualified nurses and health care assistants providing nursing care for patients requiring visits specifically within the evening and night. This may be routine care that patients may need if they are housebound or unscheduled visits to prevent hospital admission.
How to use our service
We receive referrals from your doctor (GP) and other health or social care professionals. A health care professional will provide our number to you if they feel it may be required. We aim to respond to your call within two hours of receiving it within our working hours.
