Sussex and East Surrey Sustainability and Transformation Plan published

 

The NHS, social care and public health in Sussex and East Surrey published its Sustainability and Transformation Plan (STP) today (25 November 2016). The plan outlines how local organisations will work together to transform and integrate health and social care to meet the changing needs of the people who live in our area.

 

The STP sets out how local NHS organisations think services need to change over the next five years to achieve the right care both now and in the decades to come.

 

The proposal is for STP to be delivered by three geographical areas:

  • Costal Care
  • Central Sussex and East Surrey Alliance
  • East Sussex Better Together.

 

First Community works in the Central Sussex and East Surrey Alliance area.

 

The aims of the STP is to provide:

  • Better health and well being
  • Better standard of care
  • Better use of staff and funds
