Reigate and Horley Winter Home Hub launched to support vulnerable members of the community this winter

On Wednesday 30 January 2019 a Winter Home Hub was launched for residents in Reigate and Horley. The initiative, which lasts 10 weeks, aims to offer extra support, help and advice to members of the community who may be particularly vulnerable to the effects of winter. This could include the elderly and those who have long term health conditions.

 

The Hub was launched in partnership between the British Red Cross, Surrey County Council Adult Social Care, First Community and Alliance for Better Care and is funded by winter pressures money from Surrey County Council Adult Social Care.

 

 ‘By offering extra support to those who need it the we hope to support the local health system by prevent unnecessary hospital admissions and reducing pressure on acute hospitals’ said Lena Abdu, Improvement and Transformation Lead at First Community.

 

The Hub is available to patients registered at Smallfield Surgery, Wayside Surgery and Birchwood Medical Practice in Horley and The Wall House Surgery in Reigate. To access the service patients should phone their surgery and ask for support from the Home Hub. Neighbours, friends, family member, health and social care professionals and voluntary organisations can phone on behalf of somebody who requires support.
