What does the service provide?
Short-term nursing and therapy intervention during an episode of ill health, to prevent acute hospital admission when the patients can safely be supported at home or to support hospital discharge. We aim to maximise independence by working with patients, families and their carers. A qualified nurse or therapist will undertake a holistic assessment and agree a care plan or rehabilitation programme with the patient to ensure goals are met and support is received from the appropriate health care professional.
If long-term support appears evident we work closely with other multidisciplinary health and social teams to ensure patient care is co–ordinated and continuous.
A highly skilled team of nurses, occupational therapists, physiotherapists and rehabilitation assistants will provide short-term health and rehabilitation support in a patient’s own home.
How to refer
We receive referrals from your doctor (GP) and other health or social care professionals. To prevent acute hospital admissions we aim to make contact with you within two hours of receiving a referral. To support discharge an approximate time of first visit will be discussed with the referrer who will inform you before you leave hospital.
