What does the service provide?

We aim to prevent avoidable hospital admissions and enable patients’ discharge from hospital. We will provide a period of assessment, care and rehabilitation and will work with you to identify your own goals. We will plan to enable you to return home with any necessary support or help you to identify a future care environment to meet your needs.

Who we are

Our Bed-Based Care Support services are made up of skilled multi-disciplinary teams of registered nurses, therapists and support staff, providing assessment, care and rehabilitation for patients during or following a period of ill health, to enable our patients to reach their maximum potential. We aim to respond to your referral as quickly as possible and work with accident and emergency, hospital wards and community services.

How to use our service

You can be referred into the service from an acute hospital, by your doctor (GP) or another health care professional. We will contact you to assess your needs and will then agree with you where your care and rehabilitation will be delivered.

We work across various community locations in the East Surrey area.
