Community Neurological Rehabilitation

Specialist assessment and advice for patients and carers dealing with neurological conditions such as stroke, multiple sclerosis and motor neurone disease.

What does the service provide?

Patients are seen in their own homes or at Oxted Therapies Unit. Therapists arrange appointments with you and may set-up programmes that can be carried out by our Rehabilitation Assistants.  

Our aim

Our service aims to assess you and agree goals around daily living skills, physical recovery, communication and memory and concentration in order to maximise your independence in the community.

About the team

The Community Neurological Rehabilitation Team is a multidisciplinary service consisting of Physiotherapists, Occupational Therapists, Speech and Language Therapists and Rehabilitation Assistants. We work closely with Specialist Nurses, local GP’s and Hospitals and Neurological Outpatient services. Psychology is also available for individuals that have had a stroke.  

Length of treatment

Progress will be monitored by the team on a weekly basis and it may be necessary to have a case conference with you and your family to discuss your goals. Once the agreed goals have been achieved you will be discharged from the team. You may then be referred to outpatient therapy services or local community resources at this point.

Most patients can expect to work with the team for between 6-12 weeks (maximum). 

    Patients referred to our service must be aged over 18, registered with a local GP and require a short term goal directed programme for a problem related to a neurological condition.

    Referrals can be received from GPs, local Hospitals or Secondary Care Inpatient Rehabilitation. Most people will be referred directly via a health care professional. Patients that feel they would benefit from this service should discuss this with their GP or other health care professional to make a referral on their behalf if deemed appropriate.
