How to refer

Patients referred to our service must:

  • Be 18 or over
  • Be registered with an East Surrey GP
  • Have concerns about communication and/or swallowing
  • Be medically stable
  • Agree to the service or have a carer who can agree.

Referrals can be received from GPs, other speech and language therapists and other health and social care professionals. Self-referrals can be accepted in certain circumstances.

To make a referral please complete an Adult SLT referral form and send it via email to fchc.slt@nhs.net  

We are not currently commissioned to see patients with dysphonia (voice disorders), with head and neck cancer or a primary mental health diagnosis. Voice patients should be referred by an ENT (Ear, Nose and Throat) consultant to Sussex and Surrey Healthcare NHS Trust Speech and language therapists. Address: East Surrey Hospital, Adult Speech and Language Therapy Department, Canada Avenue, Redhill, Surrey RH1 5RH http://www.surreyandsussex.nhs.uk/our-services/a-z-of-services/therapies/speech-language-therapy/
