Community and Outpatient Services

Community services are health services provided in clinics and in people's homes rather than as in-patient in hospital.

First Community provide a wide range of services based in the local community, such as in your own home, in a community hospital, nursing home bed or in a clinic setting. We provide multi-speciality teams including specialist nursing for long-term conditions and therapies.

Audiology

Specialist assessment and diagnosis for people with hearing loss and balance problems and provide appropriate support such as digital hearing aids and specialist advice.

Cardiac Rehabilitation

Specialised assessment, exercise and education programme to patients and their families following a cardiac event, revascularisation and/or heart surgery.

Care Home Advisors

Our Care Home Nurse Advisors use an advisory, supportive and facilitative approach to enable care home staff to manage their residents’ care needs competently and confidently.

Community Matrons

Working to improve the health and quality of life of people with long-term conditions by co-ordinating care and services.

Community Nursing

Specialist practitioners who head up a team of community nurses and health care assistants working with patients and their families with acute, complex and end-of-life needs to maximise their health and wellbeing. 

Community Physiotherapy

Specialist physiotherapy rehabilitation across east Surrey to patients in their homes, residential homes and nursing homes.

Dietetics

Qualified and experienced dietitians who provide a service in the community for children and adults, including children with a learning disability.

Falls Prevention

Specialist care for patients who are at risk of falling or who have had a fall.

Heart Failure

Providing specialist nursing assessment and advice on managing Heart Failure.

Intermediate Care Team

The Intermediate Care Team is part of our responsive services who provide short-term nursing and therapy intervention during an episode of ill health, to prevent acute hospital admission when you can safely be supported at home or to support hospital discharge.

Mental Health Nurse Practitioners

If you are aged over 18 you may be referred to the service if you have an active physical health need and receiving regular care from another First Community service, and you must continue to be seen by that service in order to receive support. 

Orthotics

Assesses patients who have a weakness or deformity in a part of the body as a result of a long term condition to see if provision of an orthotic appliance e.g. splints, braces, caliper would help improve mobility and support the affected area.

Outpatient Physiotherapy

A team of specialist physiotherapists, who provide expert treatment and guidance to rehabilitate and re-educate people with a wide range of musculoskeletal disorders within a clinic setting.

Specialist support for anyone who has a confirmed diagnosis of Parkinson’s

Podiatry

Assessment and treatment of a range of foot problems for patients in particular risk of complications resulting from conditions such as diabetes, arthritic conditions, rheumatoid arthritis and any vascular disease.

Respiratory

A multidisciplinary group of specialist nurses and therapists who see patients with non malignant respiratory diseases

Tissue Viability

We are a specialist nursing service for wounds that are complex or taking a long time to heal.
