Neighbourhood navigation is coming to east Surrey!

First Community are delighted to be launching a new social prescribing service in east Surrey known as Neighbourhood navigation, which will be going live soon. The aim is to empower people to manage existing health or social issues, by accessing appropriate support.
Healthcare professionals will be able to refer patients via the Joy online platform to a range of local, non-clinical services that can help with social, emotional, and practical needs. These services might include things like joining community groups, exercise classes, taking part in arts and crafts activities, or accessing practical support like debt advice or bereavement support.
List your community groups and activities on Joy
The service will be supported by the Joy online platform where all listings will be located.
If you would like to list your community group or activity on Joy you can register with an email address and password and select add a new service or complete a request form.

We're looking for passionate individuals to join our team.
5 x Neighbourhood Navigators
Work closely with patients referred into the service to understand their needs and use the Joy platform to find the best community support that will improve their overall health and wellbeing.

Engage and build relationships with a range of local stakeholders across east Surrey promoting the key benefits of the service.
