Community services are health services provided in clinics and in people's homes rather than as in-patient in hospital.

First Community provide a wide range of services based in the local community, such as in your own home, in a community hospital, nursing home bed or in a clinic setting. We provide multi-speciality teams including specialist nursing for long-term conditions and therapies.

    Specialist assessment and diagnosis for people with hearing loss and balance problems.
  2. Cardiac Rehabilitation

    Support following a cardiac event, revascularisation and/or heart surgery.
  3. Care home advisors

    Support for care home staff to manage their residents' care needs competently and confidently.
  4. Community Matrons

    Support for people with long-term conditions.
  5. Community Nursing

    Working with patients and their families with acute, complex and end-of-life needs.
  6. Community Stroke Neurological Rehab Early Support Discharge

    Support for patients and carers dealing with neurological conditions such as stroke, multiple sclerosis and motor neurone disease.
  7. Dietetics

    Diet and nutrition support for children and adults.
  8. Falls Prevention

    Care for patients who are at risk of falling or who have had a fall.
  9. Heart Failure

    Nursing assessment and advice on managing Heart Failure.
  10. Integrated Clinical Assessment and Treatment Service (ICATS)

    Support for patients with musculoskeletal (bone and joint) problems.
  11. Intermediate Care Team

    Short-term nursing and therapy intervention during an episode of ill health.
  12. Mental Health Nurse Practitioners

    You may be referred if you have an active physical health need and receiving regular care from another First Community service.
  13. Motor Neurone Disease / Rare Neurology Practitioner Service

    Support for people with a confirmed diagnosis in the east Surrey area.
  14. Multiple Sclerosis (MS)

    Care of people with a confirmed diagnosis of MS from diagnosis to end of life.
  15. Orthotics

    Support for patients who have a weakness or deformity in a part of the body as a result of a long term condition.
  16. Parkinson’s

    Specialist support for anyone who has a confirmed diagnosis of Parkinson's.
  17. Physiotherapy

    Support for people with a wide range of musculoskeletal disorders.
  18. Podiatry

    Assessment and treatment of a range of foot problems.
  19. Pulmonary Rehabilitation

    Specialised assessment, exercise and education programme to patients who have a long term respiratory condition.
  20. Respiratory

    Support for patients with non malignant respiratory diseases.
  21. Stroke Specialist Nursing

    Information and education relating to stroke recovery and prevention.
  22. Speech and Language Therapy

    Assessment and advice to patients and carers for speech, language and communication.
  23. Tissue Viability

    Nursing service for wounds that are complex or taking a long time to heal.
  24. Urgent Community Response (UCR) Service

    Urgent community care to prevent acute hospital admission.
