About the Community Nursing Service

Our service provides nursing care, advice and support to those who are housebound and unable to attend their GP surgery.
We work with you, your family, and other healthcare
professionals involved in your care, to support your acute or long-term health needs. Your care is delivered by our team of nurses  and healthcare assistants in your own home.

Our aims

  • To provide a comprehensive assessment of your health needs and develop a plan of care with you, tailored to your needs, preferences and lifestyle
  • To provide safe and expert clinical nursing care
  • To protect your safety by wearing identification, uniform, and appropriate personal protective equipment (PPE) according to government guidelines
  • To provide information and support to help you gain the skills and knowledge to care for yourself and your condition
  • To promote your health and wellbeing so that you remain in control of your own care, enabling you to anticipate your health needs
  • To provide a responsive and proactive service to reduce the likelihood of an unnecessary admission to hospital
  • The care we provide is inclusive and reflects our organisations  commitment to honour equality and diversity.

How you can help

  • Provide a safe environment for the team to visit you.
  • If you're a smoker, please refrain from smoking during our visit
  • Please be respectful of the team. Abuse of any kind will not be tolerated, and any abusive behaviour could result in withdrawal from the service
  • Hand washing is vital to prevent any potential transmission of infection. Please support your nurse by supplying hand washing facilities
  • Provide ongoing feedback to allow us to adapt our service accordingly.

Your Community Nursing Team provide services that include:

  • Wound care: including leg ulcer and oedema management
  • Palliative support and care at the end of life
  • Medication administration and support for non-oral medications
  • Continence assessments and catheter management
  • Ear irrigation
  • Phlebotomy services
  • Equipment advice and provision
  • Contact details and referrals to other appropriate community services.

Response times

Response times will be according to your assessed need.

Examples of urgent referrals include:

  • Pain or symptom control
  • Blocked catheter
  • Administration of medication, such as insulin.

Examples of non urgent referrals include:

  • Wound care
  • Continence assessments
  • One off visits.

Sharing information

There may be times when information about your health needs may be shared with other people involved in your care. If you do not wish to have information about your health needs shared, please inform a member of the Community Nursing Team.

As part of service improvement, we may wish to contact you from time to time to gain your views on how you feel about the Community Nursing Service. If you do not wish to participate in any surveys, then please inform a member of the team.
