The service is for adults aged 18 and over who are registered with a GP in the east Surrey area and have a diagnosis of Parkinson's.
A referral from your GP, consultant or other healthcare professional, confirming your diagnosis. An appointment will then be made with the specialist nurse for Parkinson's.
Contact the service if you:
The service operates between 8.30am - 4.30pm, Monday to Friday (excluding bank holidays).
You may be invited to leave a phone message and your call
will be returned within two working days.
