You will be seen in your own home or at Oxted Therapies Unit. Therapists arrange appointments with you and may set-up programmes that can be carried out by Rehabilitation Assistants.
Our service aims to assess you and agree goals around daily living skills, physical recovery, communication and memory and concentration in order to maximise your independence in the community.

To be referred to the Community Stroke and Neurological Rehabilitation or Early Supported Discharge services you must be aged over 18, registered with a local GP and require a short term goal directed programme, for a problem related to a neurological condition.
Referrals can be received from GPs, local Hospitals or Secondary Care Inpatient Rehabilitation. Most people will be referred directly via a health care professional. If you feel you would benefit from these services you should discuss this with your GP or other health care professional to make a referral on your behalf if it's deemed appropriate.
