What does the service provide?

This team provides assessment and treatment planning for patients with musculoskeletal (bone and joint) problems. They use a range of assessment tools such as X-rays and scans and work between your GP and hospital consultant to ensure that you receive the correct treatment.

Our team

Our team consists of Advanced Physiotherapy Practitioners and GP's with special interests.

How to refer

Your GP can refer you and you can also self-refer to the service. 

To self-refer, complete and return the self-referral form  FCHC MSK Self referral form[pdf] 198KB by email  fchc.msk@nhs.net  or by post to Consort House, 5-7 Queensway, Redhill, Surrey, RH1 1YB.

  • You must be 16 years old or over to refer to this service
  • You should be seeking help for a musculoskeletal (bone, joint or muscle) problem such as back pain, arthritis or a sprain
  • You must have a GP registered within the east Surrey area to refer yourself through to this service
  • You can only self-refer for one condition at a time.

Parking: The nearest car park is currently closed so please use the pay and display in Victoria Road (Victoria Road Car Park, 129-131 Victoria Rd, Horley RH6 7AS). This is a 4-minute walk away from the surgery so please set aside enough time to arrive promptly for your appointment.
