What is the stroke review service?

The Stroke Review Service is a specialist service for people who have experienced a stroke.

The service offers the following:

  • An invitation to a stroke specialist nurse review, 4-8 months post stroke
  • An annual follow up as required
  • Information and education relating to stroke recovery and prevention.

The service aims to:

  • Work together with you and your family/ carers (with your consent)
  • Assess how you are coping
  • after your stroke
  • Provide advice regarding your stroke recovery
  • Provide stroke risk reduction advice
  • Check your blood pressure and pulse
  • Review your medications
  • Identify if you need any additional help or support to enable you to maximise your independence at home and in the community
  • Put you in contact with local support groups
  • Work closely with local GPs, hospitals and neurological outpatient services.

Who is eligible for the service?

Patients over the age of 18 who have:

  • A confirmed stroke diagnosis
  • Are registered with a GP in the east Surrey area.
