First Community launches new self-referral and information library webpage to empower patients

First Community is delighted to launch its new self-referral and information library webpage, designed to make accessing healthcare services and health information easier than ever for patients, families, and carers.
The new page provides a central hub where patients can directly self-refer to some of First Community's services without needing to go through a GP or other healthcare professional, meaning quicker access to the care and support people need, when they need it. This includes speech and language therapy, physiotherapy, audiology (hearing loss and balance) and dietitian services.
Alongside the self-referral options, the site includes a patient information library, offering easy-to-read, reliable guidance and advice on a wide range of health topics in one central place. From managing long-term conditions such as joint pain or bone problems to accessing therapy and rehabilitation services, the library aims to help people stay informed, confident, and supported in their care journey.
"By making self-referral routes and health information readily available online, we're helping patients take an active role in managing their own health and wellbeing, said Oonagh Park, Head of Communications.
"The information library features a range of information on various topics that staff, patients and carers and easily access online, print and share.
"The design features simple to use tiles, ensuring content appears on the same page to improve accessibility and user experience."
This initiative directly supports the ambitions of the NHS 10 Year Health Plan, which aims to give people more control over their own health and to bring care closer to home.
