Early Supported Discharge (ESD) Service Leaflet

The Early Supported Discharge (ESD) Service supports patients following a stroke.

Our aims are to:

  • support you when you return home from hospital after a stroke. 
  • work together with you and your family / carers to continue your rehabilitation and maximise your independence at home and in the community
  • work with other health and social care professionals, social support services, and charities in providing optimum care.

Who we are:

We are a team made up of the following:

  • Occupational Therapists
  • Physiotherapists
  • Speech and Language Therapists
  • Dietitians
  • Stroke Nurses
  • Rehab assistants
  • Clinical Psychologist.

What we do:

When you are discharged from hospital, we will make contact within 24hrs (one working day). We will continue the therapy programme you started in hospital and continue to work with you on what you want to achieve.

We can help you with:

  • Getting around, walking and using public transport
  • Working towards independence with eating, drinking, managing household tasks, washing and dressing
  • Communication including reading and writing
  • Coming to terms with disability
  • Making the most of the local community facilities
  • Memory and thinking skills
  • Returning to work (vocational rehabilitation).

How long will my therapy last?

Typically, people work with our team for 1-6 weeks (maximum) depending on your needs and goals.

Your responsibility

Therapy will only be successful if you commit to, and engage with, your rehabilitation programme. Our aim is to help you develop the skills to manage your condition independently.
