Community Parkinson’s Service

Following a diagnosis of Parkinson’s or a Parkinson’s related condition it can difficult to find the right person to help you when you need advice, support, equipment and care.

People with Parkinson’s with an east Surrey GP are offered the support of a Specialist Nurse for Parkinson’s.
 

What does the service provide?

  • Information and support for those with a diagnosis of Parkinson’s, including information about the condition and available treatments.
  • Nurse-led clinics with assessment and advice on the management of Parkinson’s
  • Medication advice and review of treatment
  • Lifestyle advice
  • Telephone advice if registered with the service
  • Onward referral and information about other services that can support you in managing your Parkinson’s
  • Education to health and social care professionals working with people with Parkinson’s
  • Follow up reviews to monitor treatment progress.

Who is eligible for this service?

The service is for adults aged 18 and over who are registered with a GP in the east Surrey area and have a diagnosis of Parkinson’s.

How do I access the service?

A referral from your GP, consultant or other healthcare professional, confirming your diagnosis. An appointment will then be made with the specialist nurse for Parkinson’s.

When should I contact the service?

Contact the service if you:

  • Have a question about your Parkinson’s or treatment options
  • Are concerned by any symptoms you are experiencing
  • Are finding it difficult to cope with any aspect of your Parkinson’s
  • Need advice about your medication
  • Want further information about Parkinson’s
  • Are admitted to hospital either due to your Parkinson’s or for
  • any other reason.

Service hours

The service operates between 8.30am - 4.30pm, Monday to Friday (excluding bank holidays).
You may be invited to leave a phone message and your call
will be returned within two working days.
