Patient Safety Incident Response Plan 2026–2027

First Community has published its Patient Safety Incident Response Plan (PSIRP) for 2026-2027, outlining how the organisation will respond to patient safety incidents in a way that supports learning and quality improvement. 

The plan aligns with the NHS Patient Safety Incident Response Framework (PSIRF) and focuses on the responses that help us understand what happened and why, rather than attributing blame. It reflects our continued commitment to building a strong learning culture across all services.

Introducing the plan, Jon Ota, Chief Nurse and Director of Quality and People, said "As we look ahead, our focus will remain firmly on our learning culture, one where colleagues are supported to speak up, where insights are valued, and where improvement is driven by real world experience and compassion for both patients and staff.

"We will continue to use our safety huddle as a dynamic space to identify emerging themes, respond to risks, and celebrate positive practice. Together, we will ensure our learning responses remain timely, proportionate and grounded in the principles of the Patient Safety Incident Response Framework."

The PSIRP sets out how patient safety incidents are identified, how learning responses are agreed, and how patients, families and staff will be involved and supported throughout the process. It builds on the success of our safety huddle approach and our ongoing work to embed openness and shared learning across First Community.
