About the service

First Community's Neighbourhood Navigation service provides social prescribing to connect you to local non medical community-based services to enhance your overall wellbeing. 

There are many daily challenges which may impact your health and wellbeing which are often not medical. Stress, financial challenges, social isolation, loneliness, and many other experiences can have a negative impact on your health, and prescribing medications isn't always the answer.

The service aims to reduce pressure on other NHS services by decreasing GP and hospital visits.

How to access the service

The service is available to anyone aged 18 or over who is registered with an east Surrey GP practice. You can download a map showing the east Surrey GP practices here:East Surrey Neighbourhood map.pdf[pdf] 189KB or search for a local practice.

Self help: Joy online marketplace

You can access the Joy platform to search for your own support to improve your own wellbeing; this could be joining a group, a club or activity of your choice.

Referral 

You may benefit from more personalised 1:1 support, particularly if you face barriers to accessing community services independently, and referrals can be made to the service by your GP or other healthcare professional.

Once you're referred to the service you will be contacted by a neighbourhood navigator who will spend time with you to understand your needs and be able to either signpost or refer you directly into a local service that can support you. The navigator will be there to help you make that first step and support you through the process.

GP and healthcare professionals should make a referral via EMIS using Joy Connect. If healthcare professionals have any questions about the service the team can be contacted via fchc.neighbourhoodnavigation@nhs.net.

The team

The team consists of five neighbourhood navigators and one communications & engagement officer. 

The neighbourhood navigators work closely with you once you are referred into the service to find the best community support to improve your health and wellbeing.

The communications and engagement officer engages and builds relationships with a range of local stakeholders across east Surrey promoting the key benefits of the service.
