Our aims are to:

  • support you to achieve a good quality of life, maximise your independence and wellbeing, and help to reduce long-term disability.
  • work together with you and your family / carers to continue your rehabilitation and maximise your independence at home and in the community.
  • assess your needs and agree goals in areas such as daily living skills, physical recovery, communication and thinking skills.

Who we are:

The Community Stroke and Neuro Rehabilitation team is made up of:

  • Physiotherapists
  • Speech and Language Therapists
  • Dietitians
  • Occupational Therapists
  • Rehabilitation Assistants
  • Specialist Nurses.

We work closely with local GPs, hospital teams and neurological outpatient services. A neuropsychology service is also available for people who have had a stroke.

What we do:

We provide rehabilitation for people with a neurological condition. Therapy may take place in your home, at Oxted Therapies Unit, your workplace, a care home or in the local community.

Therapists will arrange sessions with you and may set up programmes which can be carried out with a rehabilitation assistant or independently.

We can consider goals related to:

  • Moving around your home, walking and using public transport
  • Eating, drinking, managing household tasks, washing and dressing
  • Communication including reading and writing
  • Coming to terms with disability
  • Making the most of the local community facilities
  • Memory and thinking skills
  • Returning to work (vocational rehabilitation).

How long will my therapy last?

People work with our team for 1 to 12 weeks (maximum) depending on your needs and goals.

Hours of service:

We provide services from Monday to Friday, excluding bank holidays.

Your responsibility

Therapy will only be successful if you commit to, and engage with, your rehabilitation programme. Our aim is to help you develop the skills to manage your condition independently.
