Motor Neurone Disease (MND) and Rare Neurological Conditions Service Leaflet - Information for patients and carers

The specialist practitioner for Motor Neurone Disease (MND) and Rare Neurological Conditions aims to improve the quality of life for people with the following conditions:

- Motor Neurone Disease (MND)

- Multiple Systems Atrophy (MSA)

- Progressive Supranuclear Palsy (PSP)

- Corticobasal Degeneration (CBD)

- Huntington’s Disease (HD)

The specialist practitioner can provide:

  • Information and support for people who have recently been diagnosed
  • Timely assessment and advice
  • Regular monitoring, assessment and intervention
  • A continuous point of contact
  • Onward referral and information about other services that can support you
  • Links with specialist services, charities and social care
  • Working with other health professionals to provide a multidisciplinary approach to care
  • Advanced care planning – helping people and their families talk through options and preferences for their future care
  • Teaching for health and social care staff to improve their understanding of these conditions.

Who is eligible for the specialist practitioner service?

The service is for anyone who has a confirmed diagnosis of one of the following conditions – MND, MSA, PSP, CBD, HD

They will need to be registered with a GP in the east Surrey area.

How do I get to see the specialist practitioner?

With consent, referrals are made via a neurologist, GP or other healthcare professional. People are also able to self-refer to the service.
